Three workers have died and another has been injured in a tragic accident during lifeboat maintenance on a floating storage and offloading vessel operated by Malaysian state energy giant Petronas. The incident occurred on Sunday at the FSO Sepat facility off the coast of Terengganu, the company confirmed.
Petronas said the accident took place at around 12:50 p.m. local time, with all four workers immediately evacuated. Three were pronounced dead upon arrival at hospital, while the injured staff member remains under medical observation.
In a brief statement, the company said investigations are underway “in coordination with the relevant authorities,” though no further details were provided.
Malaysia’s state news agency Bernama, citing local police, reported that the workers had boarded a lifeboat to lower themselves into the sea for maintenance work. The rope or hook supporting the lifeboat is believed to have detached, causing the victims to fall into the water.
The incident has raised serious safety concerns around offshore maintenance operations, particularly on ageing FSO platforms.
